Impromptu WalksI hereby give permission for my child to go on impromptu walking field trips in the neighborhood. This includes walks around the block or to Endion Park (1602 E 2nd st. Duluth, MN). This includes using Endion Park as a playground and includes your child playing on the playground equipment of Endion Park.SunscreenI hereby give permission to the staff at Chester Creek Montessori to apply sunscreen to my child as necessary. Your consent to the use of the photographs, video and sound recordings and your child’s image, likeness, appearance, and voice is for forever. You will not receive compensation for the use of your child’s image, likeness, appearance, and voice now or in the future.
